When do we get forms to allow someone else other than a parent to be our chaperone?

Me and a friend of mine registered for Infinitus last night, we will both be 17 at the time of the convention (a few months shy of 18) and will be chaperoned by her mom. Since she is not my legal guardian when will we receive forms for my parents to sign stating she is my guardian during the convention? Will this be given out at the hotel when we check in?

The chaperone forms will be up on the web site in plenty of time before the event to have them signed and notarized before attendees leave home. Way before.
